Gluteoplasty

(also called buttock implants surgery)

Buttock implants surgery was invented in the 1980s in Brazil. Dr Horn introduced this technique first to France (1997) and then to the UK (2000)

 

The best way to perform an augmentation of the gluteal area is with buttock implants. It will provide a projection of the top and middle thirds of the buttocks, much more noticeable than with a fat transfer of an equivalent volume

 

This surgery is the first option on patients who have no fat available and are seeking for an augmentation. It can be done as a single procedure, but can sometimes be combined with liposuction to the love handles, or even with fat transfer ( if available) to the sides of the buttocks

 

Dr Horn is using the best silicone gel buttock implants (Polytech) for his patients. These implants can be round or oval and come in about 6 different sizes. It is also possible to have custom made implants.

 

  • The surgery is performed under general anaesthetic, generally takes only 60 minutes, and requires an overnight stay.
  • A 5 cm long incision (sometimes a double incision) is made on the midline in between the buttock cheeks.
  • The implant is  put in an intramuscular pocket (just like in a sandwich). Then the muscle and the skin are sutured up, and a drain is left inside the pocket (for usually 24 h).
  • The incision site is covered with a dressing.
  • The patient then has to wear a compression garment for the next 4 weeks.

 

The recovery time is about 3 weeks

  • Sitting down is recommended as early as the first day, in order to sit comfortably a week later.
  • Sleeping is face down for the first 2 weeks and then on the sides, from the third week onward.
  • Going back to work: usually 3 weeks.
  • Going back to the gym: usually 6 weeks.

Fat Transfer to Buttocks

(also called Brazilian butt lift)

When a patient wants a gluteal augmentation without using implants, Dr Horn will recommend a fat transfer.

 

This surgery has a bad reputation as, sadly, there have been fatalities in the past in different parts of the world after such an operation. International societies of plastic surgery (ISAPS, but also BAAPS) recommend either not to do this surgery or to follow strict guidelines (avoiding injection of massive amount of fat, avoiding injections of fat into the gluteal muscle, avoiding the injection of the lower third of the buttocks).

 

  • This type of surgery is usually performed under general anesthetic but can sometimes be done with a sedation. It is a day case surgery.
  • Fat is harvested from all the areas where it is available: the flanks (love handles), the abdominal wall, or sometimes the thighs.
  • Then this fat is washed and processed, sometimes, mixed with stem cells and then carefully injected through 2 mms long incisions to the top , middle thirds of buttocks and the pertrochanteric areas (hip dips).
  • All the incisions are stitched up and small dressing applied.
  • Then the patient has to wear a compression garment for the next 3-4 weeks.
  • Sleeping should be face down for the first 2 – 3 weeks.
  • Sitting on a special pillow is allowed as early as the first day.

 

It is important to understand that some of this fat, even with the best technique, will get reabsorbed (up to 20 %). Therefore a second fat transfer session is sometimes necessary after a few months.
